Account Amendment Authority


Use this form to add or remove authorised persons (account signatories or CDF Online users), close transaction or savings accounts, and /or update account holder contact details

What is this guide for?

This guide explains how to complete the NAB Specimen Signature Card.

Why would you do this?

For CDF Cheque account signatories who require identification for NAB cheque link accounts.

What you will need

Your personal details.

Eligibility:

  • This form is available to the clients of CDF (Catholic Development Fund)
  • This form requires an electronic signature, signed on screen or using a mouse
  • If this is not suitable, please download a manual form

Identification of Authorised Person(s)

Authorised Persons will require identification when:

  • Adding a new account signatory or CDF Online user who has not previously provided identification to CDF, or
  • Adding an existing account signatory or CDF Online user who has been asked to update their identification.

 

Step 1 – Complete a Certificate of Identity

Step 2 – Complete a NAB Specimen Signature Card form

Step 3 – When the application is submitted, CDF will send an SMS containing a unique link to each authorised person requiring them to complete electronic verification of identity using their mobile phone or tablet


Important information

It is recommended that each authorised person completes and provides copies of the (1) Certificate of Identity form and (2) NAB Specimen Signature Card, before you submit the Account Amendment Authority form.


You will be required to upload these identification documents before you submit the application.
